Benefits Counselor
You can contact a Benefits Counselor through the CSB's Member Advocacy hotline. Member Advocates are available Monday-Friday, 8:30 am to 5:00 pm. Please call 800.563.9929 or submit a request online.
Plan Elections and Changes
There are two times that changes may be made to your health and life insurance elections.
- Open Enrollment: Health and Life benefits have an open enrollment annually allowing for changes in enrollment and beneficiary information.
- Special Enrollment Notice for changes occurring based on qualifying life event changes:
IRS 125 prohibits you from changing your enrollment during the Plan Year unless you experience a qualifying life event. A qualifying life event includes a marriage, divorce, death of a spouse or a dependent, birth or adoption of a child, termination or commencement of employment for your spouse, a change in employment status (full-time to part-time or part-time to full-time) for you or your spouse that affects benefits eligibility, or taking an unpaid medical leave of absence by either you or your spouse. Please keep these IRS rules in mind when completing your enrollment forms.
If you have a qualified life event and would like to change, enroll, and/or waive coverages all supporting documentation as well as your benefit elections must be turned into the Human Resources Department within 31 days of the effective date of the qualifying event.

Medical Benefits
Center For Family Services offers medical and prescription benefits to all full-time staff on the first of the month following completion of 30 days of full-time employment. You may also make changes to your benefits at open enrollment/renewal (mid June) or when experiencing a qualifying life event.
Three medical insurance plan options are available to you through Cigna.

Dental Benefits
Full-time Center For Family Services employees are eligible for dental insurance benefits through Delta Dental NJ on the first of the month following 30 days of employment. Additionally, you are able to make changes to this election at open enrollment annually, however, you may be considered a "late entrant" and subject to waiting periods for certain benefits.
Vision Benefits
All full-time Center For Family Services employees are eligible for vision benefits through Vision Benefits of America (VBA) on the first of the month following 30 days of employment or at open enrollment.
VBA represents one of the most comprehensive networks of Optometrists, Ophthalmologists, and well known Retail Optical Stores in the nation. You can search for providers by visiting Vision Benefits of America.To see a VBA provider, you must request a Benefit Form on the VBA website or by calling or by calling 1-800-432-4966. Benefit Forms are required at your Eye Appointment to ensure proper payment. You must also provide our Group Number, VBA 2108.
