Employer Paid Life Insurance
Basic Life Insurance, Long Term Disability Insurance and Accidental Death Insurance coverage is available at no cost to you - Center For Family Services pays 100% of the Basic LIfe, AD&D, and LTD premium.
All active, full-time employees working at least 35 hours each week are in enrolled in Basic Life, Accidental Death & Dismemberment (AD&D) and Long-Term Disability (LTD) plans.
- Basic Life Insurance/ AD&D benefit is one times annual salary to a maximum of $75,000.
- LTD coverage is equal to 60% of covered monthly earnings to a maximum of $5,000 per month.

Voluntary Life Insurance
Employees who work 20 or more hours weekly have the opportunity to purchase term life insurance for themselves, their spouse, and their children through SunLife Employee Benefits. Life insurance coverage is available in $10,000 increments from a minimum of $20,000 to a maximum of 5 times your annual salary not to exceed $250,000. Employees can also purchase Accidental Death & Dismemberment coverage at an additional cost.
If you elect life insurance coverage for yourself, you can buy up to 50% of that amount for your spouse in $5,000 increments up to a maximum of $120,000. If you elect coverage for yourself, you can also purchase life insurance for your child/ren (to age 19 or age 25 if full-time student) of $1,000, $5000, or $10,000.
For newly eligible employees, your opportunity to have medical questions waived on coverage up to $150,000 for yourself or $50,000 for your spouse is the first of the month following 30 days of employment. You can enroll in June during open enrollment for benefits beginning July 1. You will be subject to medical questions for any amount of coverage.
Updating Your Beneficiary
If you would like to add/change your beneficiaries, you can make changes any time directly through E3. Follow the path My E3 > Benefits > Agency Paid Life - click on Edit button on the right, and make changes accordingly.
