The Comprehensive Counseling program's professionally trained counselors are dedicated to providing support to individuals dealing with:

Intake Forms
During a traditional office visit, you would complete an intake packet that contains the necessary consent forms. For your convenience with teleservices, all of the necessary consent forms are located below. The staff at Center For Family Services will review each of these consent forms with you and your counselor can answer any of your questions. They will be located on the website for your reference when you need them. If you are not able to sign the documents electronically, your counselor will walk you through the process. They will document your consent, and will explain that all forms will be signed upon the first in-office visit.
The forms below apply in specific situations, which will be reviewed with you by your counselor. These forms are used as needed.
